Allied Health Professional assessments

In-Home assessment
Is done by Occupational therapists, chiropractors and kinesthiologists, registered nurses. These assessments evaluate personal care, housekeeping, caregiver and non-earner benefit issues in the home settting.

FAE assessement
Is an objective evaluation of a person's physical capacity in both the workplace and home setting. It is best done with detailed information being available to the assessor such as workplace physical demands analysis or a home-site assessment.

Job-Site assessment
This is an assessment done of the workplace for purposes of determining the specific physical demands of the job, or to determine what modifications might be possible or to determine ergonomic workplace modifications to enhance site specific employability of the client. The specific purpose for the assessment should be outlined.

Future Cost of Care estimates
This is a prepared report which reviews the available medical information and costs out the recommended treatment/rehabilitation requirements on a one time and annualized basis.

Psychology
This is an evaluation of a person's mental health status which involves objective metric evaluation. A Psychologist is a PhD who has also undergone further training with respect to clinical evaluation and treatment. A Psychological associate is a Master's level professional trained and licensed to do clinical assessments and treatments in the province of Ontario. Psychotherapy conducted by these clinicians is not covered by OHIP unless provided in a hospital.

Pediatric Psychology and Neuropsychology
This is a Psychological or Neuropsychological assessment done for a person who is under 18 years of age.

Neuropsychology
Is a psychologist who has undergone further specialty training with respect to central neurological conditions such as brain injury and dementia. They are specifically trained to evaluate (and treat) conditions which involve memory and concentration problems. These types of difficulties can be caused by many factors and often the assessments do not demonstrate specific injuries as the reason for the problem. Unless there is a very clear history of brain trauma, they should only be considered after a Psychology assessment has been done or there has been a recommendation by a medical specialist for this specific assessment. The assessments generally take a full day (or more) to complete in terms of the time spent with the client.

Psychovocational
This a type of report that is done by a psychologist and addresses vocational options that could be considered in the context of a person's mental health difficulties. They should only be considered when there is clear evidence that the mental health condition is the significant barrier to work capability but the potential is thought to be there for alternate types of work. In other circumstances a Vocational assessment should be considered. (See Vocational assessments)

Vocational assessment(including transferable skills analysis)
Is a health care professional who has undertaken extensive training a certification in vocation evaluation and assessments. These assessments should be considered in order to determine if there is a capacity for employment or training in an alternative line of work given a certain set of physical limitations. These assessments generally also include a transferable skills analysis which reviews the client's prior training and experience and determines what of that could be translated into alternative vocational areas (if any). Another component that can be done is a Labour Market survey whereby a particular geographic region is researched to determine what types of jobs are available consistent with the recommended types of jobs from the vocational assessment. This is not done routinely as part of a Vocational assessment. It has to be specifically requested and increases the cost of the report.

Social Work
Assessment done with respect to family and support dynamics, coping capabilities and review of available community support services where required.

Medical/Surgical assessments

Physiatry
Is a specialist in Physical Medicine and Rehabilitation - focusing on nuerological and musculoskeletal problems both from a diagnostic and long term management approach. Many also engage in Chronic Pain Assessments and treatments. They may also provide EMG diagnostic services.

Neurology
Is a specialist in conditions affecting the brain, the spinal cord and the peripheral nervous system. They will evaluate head injury, dizziness and headache problems as well as range of conditions affecting the nervous system including the peripheral nervous system. This would include pain conditions associated with nerve damage. Many neurologist also offer EMG diagnostic services.

Rheumatology
Is a specialist in arthritis and inflammatory conditions affecting the body. Certain soft tissue conditions such as fibromyalgia have historically been in the domain of expertise of rheumatologists although many other specialists can also diagnose and address this condition.

Gastroenterology
Is a specialist in medical conditions affecting the digestive system (esophagus, stomach, bowels, pancreas, liver and gall bladder). They are often asked to evaluate circumstances of persistent abdominal pain.

Psychiatry
Is a physician specialized to diagnose and treat mental disorders. They typically are involved in the major mental disorders such as schizophrenia and bilpolar disorder, but can evaluate and treat a range of mental disorders as well as prescribe and monitor (and comment on) medication for mental disorders and other forms of treatment. They can also admit and treat people in hospital if required. Some psychiatrists also deal with chronic pain patients.

Neuropsychiatry
This is a highly specialized physician who is cross trained as a specialist in both Neurology and Psychiatry. There are very few and the wait list for assessments is generally very long (as well as very expensive). They tend to deal with cases where a neurological condition has resulted in significant behavioral and mental health issues, such as brain trauma with a marked change in behaviour or severe behavioral changes that have occurred as result of other trauma or medical conditions. These assessments should only be done on a very selective basis and on specific recommendation or review.

Orthopaedic
Is a surgical specialist dealing with conditions involving bones, muscles, joints and spine. They can and do assess a wide range of musculoskeletal conditions as well as provide surgical treatment.

Pain Medicine
Is a specialist physician who has taken on a practice in chronic pain as part of their clinical delivery spectrum. This is typically a specialist trained in another area. Those areas are most likely to be Anaesthesiology, Physiatry, Neurosurgery, Rheumatology or Psychiatry.

Plastic Surgery
Is a surgical specialist in cosmetic and reconstructive surgery. They typically address the issue of deformities that may be trauma related for example, such as trauma related scars which are deforming either in terms of function or appearance.

Opthalmology
Is a specialist in disorders of the eyes. They assess conditions that have affected the visual system including trauma related conditions.

ENT
Is a surgical specialist in conditions affecting the ear, nose and throat regions of the body. This includes sinus conditions and facial fracture/deformity conditions as well as trauma related hearing loss and ear conditions resulting in dizziness and loss of balance.

Is a surgical specialist specializing in conditions affecting the kidney and bladder as well as prostate conditions and erectile dysfunction in men.

Dental/TMJ
Is a clinician trained in dentistry. Most dentists can do an assessment with respect to dental injury and Tempormandibular joint conditions. In some cases a subspecialty dental assessment is required but should be recommended by a general dental assessment.

Neurosurgery
Surgical specialty dealing with disorders affecting the brain, spine and peripheral nerves. Provide surgical treatment options. May be involved in chronic pain management.

Medical Investigation Services

MRI assessments
MRI's can be procured as a third party assessment, in many cases. They should not be requested except on the recommendation of a health care professional. A physician is required to complete the requisition for the hospital or facility to do the test.

Nuclear medicine assessments
Specifically bone scans can also be requested as a third party assessment in many cases. The criterion are also that it should be a specific recommendation by a health care professional and the actual requisition needs to be signed by a physician.

EMG assessments
When EMGs are recommended, these can also be obtained as a service. These are tests to determine nerve pinching or nerve or muscle damage, either in the spine region or in the arms or legs.

Catastrophic Impairment assessments
Coordination, advice and compilation of assessments to determine if a client meets catastrophic impairment threshold. Includes a whole body impairment percentage determination where applicable.

Rebuttal reports (Section 42.1)
We provide a full range of rebuttal reply reports including in reply to post-104 disability determinations and Section 42 catastrophic impairment assessments. Original provider limitations sometimes apply and replies are not covered in response to Section 42 assessments of OCF-22 applications.

Expert file review
We provide a review of medical brief or telephone consultations about files from a medical legal perspective in order to provide input as to what might be helpful with respect to managing a file.
